“Ours is a humanitarian mission,” stated Pete Joiner, CEO of Madeira Therapeutics, a business started in Kansas three years ago focusing only on pharmaceuticals for children. 70% of the prescriptions for children, in fact, are not approved by the authorities. “The goal is to create formulations adapted to those under 18 years old and approved by the FDA and EMEA, especially those that are difficult to swallow. All while keeping an eye on generics.” From the US to Europe, the distance is short: “We are just concluding negotiations for a development program in Spain.”

11.13.09: Studies suggest overweight kids are coronary time bombs
CNN recently reported that, with childhood obesity on a disturbing rise, children are becoming coronary time bombs. The evidence is mounting that children are trading playing outside for television and video games. At Madeira Therapeutics, we understand that there is a combination of circumstances contributing to the childhood obesity rate, which has tripled since 1970. We have found heterozygous familial hypercholesterolemia (HeFH) preconditions some children to develop cardiovascular complications, while a less active, more fast food-minded culture has worsened the issue.
While education regarding the urgency of this situation is intensifying, medical professionals are currently burdened with the lack of appropriate treatment options for its complications. Physicians are often compelled to resort to off-label prescriptions of adult formulations, a practice that lacks suitable dose titrations and standardized regulations.
Madeira Therapeutics aims to alleviate some of the pressure by developing a liquid statin to lower cholesterol in children when diet and exercise are not enough. We strongly believe that exercise and education are key components to curbing the obesity epidemic. However, since a recent American Heart Association study likened the arteries of obese children to those of 45 year-olds, it is clear that efficacious treatment options need to be available for this trend to turn around.

10-27-09: The Need for Pediatric Liquid Formulations
When liquid Tamiflu for children ran short recently, the FDA authorized pharmacists to mix their own liquid version of the drug using expired Tamiflu capsules and a commercial cherry syrup. The debacle occurring with the country facing its most challenging flu season in years underscores Madeira Therapeutics’ mission: more drugs need to be available in true pediatric formulations to treat children.
With only a quarter of pharmaceuticals labeled for pediatric use, physicians often are forced to write off-label prescriptions. However, using extemporaneous formulations with untested bioavailability and stability in children is clearly a dangerous practice. Doesn’t it make more sense to accelerate programs to bring important medicines to market that are intended and labeled for pediatric use? Using the 505(b)(2) approval pathway, the FDA allows adult drugs to be expediently reformulated for children. Madeira Therapeutics has determined a fast-track development timetable for three liquid formulated pediatric products.
I am hopeful the Tamiflu liquid shortage will open the industry’s and consumers’ eyes to the urgent need for specialized pediatric drugs. Madeira Therapeutics is one of the few drug development companies with a dedicated focus on this niche market − pediatric formulations that are sized to fit.
